Running through the features as a list is the cleanest way to make sense of them, so in Mental you’ll encounter Fire Frames, Dead Patients, Enhancer Cells, Mental Transformations, Autopsy Free Spins, Lobotomy Free Spins, Mental Free Spins, !!! This is Mental !!!, and 4 feature buy options.
In the base game, on every spin, 1 to 13 positions are randomly surrounded by flames. These are known as Fire Frames, and any symbol that lands inside one splits into 2 parts.
The foreboding Dead Patient feature activates when 2 Dead Patient symbols land. It reveals 1 of the premium patient symbols with a random multiplier of x5, x10, x15, x30, x50, x100, x300, x500, x1,000, or x9,999 attached. The multiplier value corresponds to the number of that symbol on the reel rather than acting as a win multiplier.
Located at the bottom of reels 2 and 4 are the Enhancer Cells. Reel 2 and reel 4 become active with 4 or 6 Fire Frames, respectively. When switched on, Enhancer Cells can reveal one of the following effects:

Hitting spider symbols on reel 2 or 4 triggers the Mental Transform feature. Spiders transform into either xWays, xSplit, Dead Patient, Wild, or Patient symbol.

Now for the free spins. The first set, 8 Autopsy Free Spins, is triggered when 3 Scorpion symbols land on reels 1, 3, and 5. Any Scorpion symbol that lands inside a Fire Frame grants +1 spin. Mental Free Spins
The top-paying bonus triggers in the main game when 3 Scorpion Symbols and 2 Spider Symbols land, awarding 10 spins. The key difference is that both Spider Symbols are sticky, causing Mental Transformations on each spin. In addition, the Dead Multiplier count does not decrease after its multiplier is passed onto new Patient symbols.

Mental: Slot Verdict
Mental absolutely earns its title, no debate. There are very few games out there like it—if any at all—and you can see why. Mental is bleak, and it’s not built to raise a laugh unless your humour runs in unusual directions. That means it doesn’t deliver the same “fun” energy as other Nolimit releases this year. There’s none of the upbeat East Coast vs West Coast swagger, the playful chaos of xWays Hoarder xSplit, or the dark comedy in El Paso Gunfight xNudge and San Quentin xWays. This is heavy subject matter, handled in a heavy way. Maybe with a theme like this, going light isn’t really an option. When you’re done playing, you may want something to reset your brain—like watching an episode of Keeping up with the Kardashians after a horror film. It’s amusing how returning to slots like Immortal Romance, A Dark Matter, or Mysterious suddenly feels like Barney the Dinosaur teaching the four times table.
Another potential sticking point is how complex the bonus rounds are. No one doubts Mental can explode when the right symbols show up, but it does edge into “a bit too complicated” at times. Trying it in demo is basically mandatory, and even then you’ll need a solid session to catch all the little details. Some players will love that depth; others will bolt from the asylum and head for the nearest fruit machine.
Looking at Mental’s paytable, it’s easy to imagine a circle of bearded Victorian gentlemen with pipes poring over the rules, trying to work out what’s happening before giving up and inventing natural selection instead because it’s clearer. Jokes aside, Mental isn’t so terrible once you take the time to learn it, but without that effort it’s close to unreadable. There’s so much splitting, slicing, transforming, locking, and more that it can feel overwhelming at first. It’s undeniably smart, but it also pushes right up against the boundary of overstuffed, kitchen-sink design.
All in all, yes, we’ve been a bit tongue-in-cheek, but Mental is about as dark as slot themes get. If you’re okay with the topic, the uneasy visuals, and the unwieldy rules, you’ll find a truly unique game here—one that stretches the limits as much as anything Nolimit City has put out.
